A Clinical Study on Vertebrobasilar Arteriography


Abstract
The purpose of this report is to evaluate vertebrobasilar arteriography as a diagnostic tool in neurosurgery.
It is based on 45 successful vertebrobasilar arteriograms obtained at the Department of Neurosurgery, Woo Sok University Hospital.
The method used and the complications observed will be presented, and an analysis of the case will be given. To simplify the technique of. vertebrobasilar arteriography, author devised a special needle for direct puncture of the vertebral artery in the neck. The needle has a side opening at the tip, and notch on the side of the needle hub indicates the directionof the small opening.
Of 45 cases with successful vertebrobasilar, arteriograms, 34 cases were explored by suboccipital craniectomy to confirm the posterior fossa lesions. The results were as follows;
1. Direct vertebral puncture technique by anterior cervical approach had the advantages of simplicity and ease of performance since adoption of the special needle.
2. Of 50 attempts at direct vertebral puncture technique, 45 were successful (90%).
3. There were 6 complications, one of which consisted of dysarthria and shouIder weakness as a permanent complication in the entire series of 50 patients with both successful and unsuccessful direct vertebral puncture technique. 4. The vertebrobasilar arteriography had a great value in the diagnosis and localization of neoplasms or vascular diseases of the posterior fossa, but not of traumatic posterior fossa lesions.
